Our Path Our Journey Your: Youth Referral Form

Our Path, Our Journey is a healing-centered, faith-rooted program designed to support Black girls through identity development, emotional wellness, and personal growth. This referral form allows trusted adults (educators, school staff, counselors, community workers, mentors, and youth advocates) to recommend a young Black girl who may benefit from this program.
